I have been working out for years, but unfortunately, it has been a while since I have worked out. I have some shoulder and elbow problems that like to flair up when I start exercising, so I am seriously looking into starting this workout, P90, very soon. I guess I know where I can find the P90 worksheets!

It's a rare form of shingles, and it does not sound fun.

Trainer and P90X founder Tony Horton’s Instagram account is filled with high-energy workout videos and inspirational messaging, but a recent post was definitely different. In it, Horton reveals that he was recently diagnosed with Ramsay Hunt Syndrome, a complication of shingles.

“What started as a perfectly normal day ended up with a trip to the emergency room and a diagnosis of Ramsay Hunt Syndrome,” he wrote in the post on Wednesday. “This battle has taught me quite a few lessons; many lessons I thought I had already learned, and thought I was putting into practice.”

“Shingles is caused by stress and apparently I’ve been stressed and wasn’t even aware of it,” he continued. “Now it’s time for me to look at how I suppress my emotions and try to fight through everything with exercise alone.”

Horton tells SELF that he first noticed something was off about three weeks ago when he had some pain on the right side of his head, followed by a tingling in his ear. “By the next morning, I noticed I didn’t have the ability to whistle and there were some odd things going on with my face,” he says. Still, he thought maybe he had pinched a nerve in his neck.

A few days later, he realized he drooled when he tried to drink water and couldn’t close his right eye. “The pain and burning got worse,” he says. He went to the ER, where doctors determined that he had Ramsay Hunt Syndrome. He was discharged but later went to the ER again when his symptoms got worse. “I’ve had balance issues, vertigo, intense pain, vomiting—I was literally vomiting in the waiting room while they were trying to get me in a bed,” he says. “This is the most intense thing I’ve been through and I’ve broken legs and shattered kneecaps. It’s relentless and there are so many things going on at once.”

Now, Horton is on anti-viral medication and steroids, but he’s still dealing with vertigo, his right eye doesn’t blink at the same rate as his left, and he has Bell’s palsy (sudden weakness on one side of the face). So, although things are slowly getting better, he’s still not feeling great. “I’m not even close to being able to drive or work out and I have to take cat naps all day,” he says. “I have little moments of energy and then have to lay down.”

Ramsay Hunt Syndrome is a rare form of shingles that affects the facial nerves.

Although shingles is fairly common, Ramsay Hunt Syndrome isn’t. About one in three people in the U.S. will develop shingles at some point in their life, according to the Centers for Disease Control and Prevention. Shingles is a viral infection that’s caused by the varicella zoster virus, which also causes chickenpox. If someone has had chickenpox at some point in their life, they’re at risk of developing shingles down the road. Anyone can get it, but the risk is especially high among people who are 60 or older—about half of all cases of shingles happen in that population—so it's recommended that people at that age get the vaccine.

Shingles most commonly affects the nerves in the abdomen and shoulders, but it can also spread to the eyes and face. If it does get to the facial nerves, it can cause Ramsay Hunt Syndrome, which causes some “pretty unique symptoms,' Amesh A. Adalja, M.D., a board-certified infectious disease physician and affiliated scholar at the Johns Hopkins Center for Health Security, tells SELF. In addition to the characteristic rash, those include facial paralysis, vertigo, nausea, vomiting, and ringing in your ear.

Most of the symptoms are temporary, but some—including hearing loss and ear pain—may be permanent, William Schaffner, M.D., an infectious disease specialist and professor at the Vanderbilt University School of Medicine, tells SELF. If someone has difficulty closing their eye for a long period of time, it can cause dryness which can damage the cornea, he adds.

It's not always clear why some people develop shingles and others don't. But Dr. Adalja says that stress definitely plays a role because it can dampen the immune system, giving the varicella zoster virus a chance to reactivate in the body. Interestingly, stress doesn't really determine which form of shingles you get. Unfortunately, doctors really just don’t know why some people get “regular” shingles while others get Ramsay Hunt Syndrome, Dr. Schaffner adds.

Most symptoms of Ramsay Hunt Syndrome are treatable.

People who develop the condition will usually be given steroids and anti-viral medication like Horton, Dr. Adalja says. It’s hard to know how quickly someone will recover—some may take weeks while others can take months—but people usually do get better with no lingering side effects. Dr. Schaffner is hopeful that Ramsay Hunt Syndrome will be obliterated it the future thanks to a promising new shingles vaccine, which has been recommended by a CDC advisory panel as of this week. “It looks to be extraordinarily successful,” he says.

As for Horton, he’s just focused on getting better. “This has been a massive wake-up call for me,” he says. “I’m pushing and traveling all the time. It makes you realize that maybe I need to back off a little, get more and better sleep, be more in the moment, and kick back a little more.”
